When you have completed the New Client Intake and want on-going accountability and support, a quick follow-up to tweak your plan, or help with an acute need like a stomach bug or seasonal allergies, Health & Hearth offers a few options:
Ongoing Support Packages
I offer 3 types of packages to provide the best care possible. Packages include a combination of in-depth consultations (up to 2 hours), follow-up appointments (up to 30 minutes), quick check-ins to touch base and see how things are going, and email support.
3-month
Building Health Program

Change can take time. This program gives you the depth, consistency, and support to establish habits, adjust for your specific lifestyle and health needs and help you to realize the wellness outcomes you seek.
This package includes:
Session 1: In depth discussion of health and personal goals and needs.
Between Session 1 & 2: Rivka develops Initial Health Plan delivered by Session 2
Session 2: Reviewing and editing the plan together
Follow -up every other week.
Regular support by email or phone.

6-MOnth Comprehensive Health Program

For those on a longer wellness journey, whether it’s navigating complex or longstanding health challenges or wanting to shift their wellness in bigger ways, six months gives us time and depth to try to understand root causes, not just manage symptoms.
This package includes:
6 in-depth consultations
6 follow-up consultations

As-Needed Support
$50/half-hour appointment
Already have a health plan and just need a tune-up? Have an acute illness or other more urgent need? This option gives you access to Rivka’s expertise when you need it most, no long-term commitment required.
This as-needed service is good for:

Getting extra support
Health plan updates
